Create subject access request (SAR) form

Subject access request forms allow your organization to customize online forms to collect the relevant information needed from end-users to exercise their rights regarding their data pursuant to various regulatory regimes (right to access and right to request deletion). 

In this article, we will cover how to create a new subject access request form and outline the basic navigation of the subject access form (SAR) builder.


Create new subject access request (SAR) form

Click Messages on the left-hand panel and select U.S. Privacy Messages (Legacy). Click Subject Access Request Form from the subsequent menu. 

  Note: Despite being nested under U.S. Privacy Messages (Legacy), subject access request forms can be created for any regulatory regime that requires subject access request functionality (like GDPR).

Use the property field in the upper right-hand corner to select a property for which you will create a subject access request (SAR) form.

Click + New Message to start a new subject access request (SAR) form.

Use the subsequent page to customize the subject access request (SAR) form for the property and click Create Message when finished. In the following section, we will provide a brief overview of how to navigate the subject access request (SAR) form builder.


Subject access request (SAR) form views

In addition to the subject access request (SAR) form that an end-user will use to make their request to access/delete their data, Sourcepoint's form builder also allows you to customize a number of views that an end-user may encounter. These views include:

View Description
SAR Form The form used by an end-user to submit an access/delete data request to your organization. 
SAR Success View The end-user's access/delete request was successfully submitted and a ticket with our ticketing partner was created. 
Verification Error View The end-user has reached their pre-determined limit for verification attempts. 
Max Requests Error View The end-user has reached their limit for access/delete requests. 
Active Request Error View The end-user already has an open access/delete request in the ticketing system.

To toggle between the different views in the subject access request (SAR) form builder, expand the Navigate View accordion and select the radio button inline with the view you wish to edit.

Navigate subject access request (SAR) form builder

In this section, we will cover the basic layout of the subject access request (SAR) form builder and how to customize components.

Select a view to edit. Configure the general settings for the view at any time by clicking the view name in the breadcrumbs and using the Settings panel on the right-hand side.

Click + in the toolbar panel to add a new component to the builder. To customize the component (or any other component, click the component in the builder and use the Settings panel on the right-hand side to customize the details of that component.

  Note: The breadcrumbs will enable you to quickly see which component you are currently editing.
